Small fire in pilot house. Extinguished immediately with a CO2 Fire Extinguisher; slight damage consisting of paint being burned off of an area 4' x 6' in the left rear corner of the pilot house overhead.

[8-12 Hours]
Received from Marine Barracks, Pearl Harbor, TH., with bags, hammocks, rifles & staff returns for duty.

Commenced flooding dry-dock & making preparations for getting underway. Left dry-dock, underway for Berth C-3, Pearl Harbor, TH. Pilot C. E. Carlson at the Conn., Captain, Navigator, & Executive Officer on the Bridge.
Moored into Berth C-3. Pilot C. E. Carlson left the ship.

McArthur, E. F., MM2c, V-6, USNR transferred to USS MEDUSA AR-1 for duty with bag, hammock, records & accounts.

Ford, John, Comdr. D-V(S), USNR, in charge of Newmark, Stephen M., P2c, V-6, USNR reported on board for temporary additional duty.

Moored in Berth C-3, Pearl Harbor, TH.

[12-16 Hours]
Polk, Howard M., F1c, USN returned aboard having received injury to right arm while on authorized liberty, negligence not apparent. Diagnosis: Fracture, simple, distal end radius & ulna, right arm. Cause of Injury: While roller skating on morning of this date the man slipped and fell, and as he fell, he extended his right arm to protect himself, thus causing the above injury. He was not intoxicated and was not in misconduct. Treatment: x-ray examination made, plaster of Paris splint applied, and patient was put to bed, then transferred with bag, hammock, records & accounts to the US Naval Hospital, Pearl Harbor, TH., for further treatment.

Gallagher, S. J., PhM1c, USN was Honorably Discharged from this vessel and the Naval Service, by reason of expiration of enlistment, and for the convenience of the Government. He re-enlisted on board March 16th, 1942, on this vessel, for four (4) years.

Made & completed preparations for getting underway. Got underway at 0754. Captain at the Conn., Executive Officer & Navigator on the Bridge. Standing out of Pearl Harbor, TH.

Commenced maneuvering to form column astern USS NORTHAMPTON. Set conditions of Readiness III throughout the ship.

Steaming in Hawaiian Operation Area. Ship in Material Condition "ZED", in Condition of Readiness III, in company with TASK FORCE 16.

[8-12 Hours]
Swim, J. K., EM3c, USN, received contusion of right knee, left foot KL"G", while climbing ladder to searchlight platform, wind tore man from ladder. Negligence not apparent. Treated at scene of accident and admitted to sick list.

[16-20 Hours]
Commenced maneuvering standing in to Pearl Harbor, TH. Captain, Exec. Officer and Navigator on the Bridge. The Captain relieved the Officer of the Deck of the Conn. Moored starboard side to port side of USS NORTHAMPTON, in Berth C-3, Pearl Harbor, TH.

[12-16 Hours]
Transferred the following men to US NAVAL HOSPITAL, Pearl Harbor, TH., with bags, hammocks, records & accounts, for treatment.

[16-20 Hours]
Billingsley, K. W., GM3c, USN received a fracture of the little toe, left foot, while covering a machine gun. The cover caught on a first aid box which fell on the man's foot and caused the above injury. Man treated in Sick Bay and admitted to the binnacle list.

[12-16 Hours]
Shaw, W. R., Lt. Cmdr., USN reported on board for temporary duty as gunnery officer of this vessel.

Heavilon, Fred F., S1c, USN pushpen received a fracture, fibula, right foot. Cause: 1. within command. 2. work 3. Negligence not apparent. 4. While walking along starboard boat skids, turned his ankle, resulting in injury described above. Treatment: X-ray taken, bandage and hot water bottle applied, and the patient admitted to sick list. Transferred on March 31st, to US Naval Hospital, Pearl Harbor, TH. for treatment.

Hunter, J. D., F1c, USN pushpen was transferred, with bag, hammock, records & accounts, to USS WEST VIRGINIA for duty.

Snead, Howard L., GM3c, USN was transferred with bag, hammock, records & accounts, to US REC. STAT., Pearl Harbor, TH. FFT to US REC. SHIP, NY, NY., for duty in Project JIG HYPO.

Holman, Richard B., CGM(AA), USN was transferred with bag, hammock, records & accounts, to US REC. STAT., Pearl Harbor, TH., FFT to US Navy Yard, Washington, DC., for instruction at the Naval Gun Factory in the type of armament being installed in the USS TAYLOR, ultimate assignment to that vessel.

Nelson, Norman M., F1c, USN was received on board for duty with bag & hammock. Records & accounts to be forwarded by registered mail.
